Special Pay for Medical Officers

Variable Special Pay: Entitlement to Variable Special Pay extends to medical officers of the Air Force, Army, Navy, and the Public Health Service. The following medical officers, serving less than a year, are eligible:

  • Reservists ordered to active duty, other than training, for more than 30 days but less than a year
  • Active duty medical officers involuntarily retained under 10 USC 12305
  • Active duty medical officers who voluntarily remain when some are involuntarily retained
  • Retirees recalled to active duty for more than 30 days under 10 USC 688.

Rates payable start at $100.00 per month for an intern and peak at $1,000.00 per month when an officer has between six and eight years of creditable service. The rates payable continue to decrease until medical officers are capped at $583.33 per month for grades above 0-6 or with more than 22 years of creditable service.

Board Certified Pay: Board Certified Pay is for medical officers who are entitled to Variable Special Pay and certified by an American Medical or Osteopathic Specialty Examining Board or board certification equivalency established for specialties unique to military medicine requiring formal postgraduate medical training of at least two academic years.

The rates payable are based on incremental increases in creditable service. Rates reange from the bottom tier (less than 18 years creditable service) at $208.33 per month to the top tier (those with 18 or more years creditable service) at $500.00 per month.

Additional Special Pay: Medical officers are entitled to receive Additional Special Pay when authorized by the military service concerned and when not undergoing an internship or initial residency training. The following medical officers serving less than a year are eligible:

  • Reservists ordered to active duty, other than training, for more than 30 days but less than a year
  • Active duty medical officers involuntarily retained under 10 USC 12305
  • Active duty medical officers who voluntarily remain when some are involuntarily retained
  • Retirees recalled to active duty for more than 30 days under 10 USC 688

The rate payable for Additional Special Pay is $15,000.00 for each 12-month period. For those medical officers entitled to this pay with less than 12 months, they are entitled to a monthly pro rated share of the 12-month entitlement.

Incentive Special Pay: Special pay for Active Duty of Reserve Medical Officers: The Special Pay was established for reserve medical officers called to active duty for less the one year for annual training, active duty for trainin, or active duty for special work. The rate payable is set at $450.00 per month or pro-rated by day for any portion of a month.

Those called to active duty for more than 30 days, but less than one year, for other than active duty training, are not eligible.

Mulityear Special Pay (MSP): Entitlement to Multiyear Special Pay (MSP) extends to medical officers in pay grades 0-6 or below who are fully qualified in a designated specialty. Additionally, medical officers planning to sign an MSP agreement must:

  • Have a current, valid, unrestricted license or approved waiver
  • Have eight or more years of creditable service, or have completed any active duty service commitment resulting from medical education or training
  • Have signed an agreement to remain on active duty for an additional two to four years
  • Understand that the military service secretary can decline to accept an MSP agreement or can reduce the agreement length to less than four years

Medical officers can be paid for any current credential specialty as long as the MSP and ISP specialty are the same.

Board Certified Pay for Nonphysician Health Care Providers: This entitlement is extended to those officers with a post-baccalaureate degree and professional board certification in their specialty. The fields authorized under this special pay include:

  • Dietetics
  • Occupational Therapy
  • Optometry
  • Pharmacy
  • Physical Therapy
  • Podiatry
  • Social Work
  • Nurse Anesthetist, Nurse Practitioner, Nurse Midwife, Adult Nurse Practitioner
  • Navy Radiation Specialist
  • Air Force Health Physicist (Medical)
  • Family Nurse Practitioner
  • Physician Assistant
  • Audiology/Speech Pathology
  • Neurorehabilitation
  • Ergonomics

The rates payable for this special pay start at $166.66 per month (for those with less than 10 year’s creditable service). The maximum payable is $416.66 per month (for those officers with 18 or more years of creditable service).

Diplomat Pay for Psychologists: Reciept of this entitlement is at the discretion of the military service secretary concerned. To become eligible, officers must have been awarded a deploma as a Deplomat in Pyschology by the American Board of Professional Pyschology.

Rate payable are tiered from a low of $166.66 per month, for those with less than 10 years of creditable service, to the maximun payable of $416.66 per month for those officers with 18 or more years of creditable service.
